Duoderm patch on cranial keel no longer present - wound is mostly resolved, did not replace. Replaced figure 8 wrap and body band. Found no casts, no scraps. Dropped 68g rat with meds on food.

O: Removed wrap from R wing. Removed bandage from sutures on dorsal wing. Small amount of bleeding started at the cranial aspect of the sutures during PT, cleaned and placed drop of tissue glue here. Able to get good extension in wing, no abnormalities felt on palpation of elbow. Took x-rays: bony healing starting, ulna fracture looks in better alignment than last week. Harder to see chip fracture at elbow as well. Fracture with callus started seen on R scapula - shoulder feels stable on palpation, but elected to add a body wrap to help stabilize this area more. Placed TAO and telfa/tegaderm on sutures on dorsal wing. Bird started gurgling after getting medications orally - seems to be aspiration risk.

P: Ok to end calcium slurry and tramadol. Change wing wrap in 3d with PT. Caution giving meds due to aspiration risk.

O: More reactive/BAR in hand today than yesterday. Small cranial keel wound bandage lifting up, elected to remove and not replace, covered spot with dermagel. Removed figure-8 wrap - large hematoma on the dorsal aspect of the wing at the fracture site. Flex/instability at the fracture site. Took x-rays: fragments are still a little displaced from each other, concerned for some signs of early osteomyelitis starting along the edges of the bone. Small chip fracture seen off the end of the proximal ulna at the elbow - can see it on the intake x-ray but it has moved and now is more obvious. Possible microfractures present at the proximal ulna as well when comparing x-rays of both wings. No crepitus felt on palpation of elbow, able to get full extension. Plucked and scrubbed hematoma on wing, applied lidocaine to area and incised swelling with #15 blade. Removed old blood clots, flushed area out, then closed with 4-0 webcryl in simple continuous pattern with tissue glue. Covered with telfa/teagerm, replaced figure-8 wrap.

A: Mentation is improved today. Chip fracture off the elbow found on repeat x-rays. Bird would benefit from surgery to better align ulna fracture but would need to seat IM pin near elbow and concerned that this damaged area could shatter more. Still has a good chance of healing without surgery, although concerned that there are now some radiographic signs of early bone infection. Elbow chip fracture does not feel abnormal at this time, but concerned for overall prognosis and risk for arthritis with this here - could possibly consider trying to remove small chip fragment but would be a risk of further damage to the elbow as well, so plan to recheck rads in a few days and reassess before going to surgery.

P: Switch from Clavamox to clindamycin. Change wing wrap in 3 d with bandage change and repeat x-rays.

O: Figure-8 wrap in place. Small flat scab on the plantar aspect of RD1, healed cere wound noted on exam. Small, wound on the cranial aspect of the keel, covered with duoderm. Bird appears visual OU, but has some retinitis OS. Mentation very QAR throughout treatment. Heart sounds are a little quiet. Drew blood from the R and L medial metatarsal vein. Reviewed intake x-rays: short oblique R ulna fracture, fragments displaced, large hematoma. Possible old L pelvic fracture but bird is rotated so not definitive finding, no crepitus or abnormalities felt on palpation.

A: Mature hatch year Red Tailed Hawk, unknown cause of trauma but sustained R ulna fracture. Radius is intact but fracture would likely benefit from surgical intervention to correct alignment of ulna. Bird a little skinny and has a minor keel and foot wound. Was previous patient 25-445 in which he had head and ocular trauma - eyes still looks the same as when he left, a little quiet so possibly has new or lingering head trauma. Guarded prognosis

P: Rx Fluids, Meloxicam, Gabapentin, Tramadol starting tomorrow, Terbinafine, Pentoxifylline, Calcium Slurry, Baytril and Clavamox. Change wrap in 3 days or when having surgery. Collect blood, fecal, and IOPs.
